Intake Process: Before your first visit, we may request brief intake forms to better understand your concerns, medical history, and treatment goals. This information helps ensure a thoughtful and personalized evaluation.
Confidentiality: All patient information is kept strictly confidential in accordance with medical privacy laws (HIPAA). Your records are protected and will never be shared without your written consent, except when required by law for safety reasons.
Cancellations & Rescheduling: We kindly ask for at least 48 hours’ notice if you need to reschedule or cancel an appointment. This allows us to offer that time to another patient in need.
